Abstract Introduction Ventral hernia repair is one of the challenging surgical operations over time. It suggested that sublay mesh has lowest recurrence and site infection in open anterior abdominal repair. This study aimed to analyse pros cons ventral evaluate significance this technique as a treatment modality. Hospital stay, acute postoperative complications, rate were main areas investigation. Methods A retrospective on 79 Patients with hernias operated (electively) 3 years. Fit patients first-time (primary incisional) included. Recurrent hernia, associated decompensated cardiopulmonary disorders, bleeding disorders excluded. The project Performa includes; patient's demographics, operative details, length follow-up, up 12 months. Results All underwent using technique. was five times more common females than males. Mean age presentation at 44.8 years old. operating time 67 minutes 1-day hospital stay. Para-umbilical incisional represented majority cases. Component separation approach added three cases (3.7%). Simultaneous cholecystectomy performed 2 (2.5%). Only 6 (6.3%) developed wound-related while (2.5%) had recurrence. Conclusion Sublay favourable for reconstruction. smooth short stay least incidence complications
